We are in search of a qualified Social Services Assistant :
To act as a liaison and representative of the residents’ social interests and to assist in planning, organizing, developing and participates in the overall operation of the Social Services Department in accordance with current federal, state and local standards governing the facility, and as may be directed by the Administrator and Social Services Director, to ensure that the medically-related emotional and social needs of the residents are met and maintained on an individual basis.

- Must have patience, tact, enthusiasm, and positive attitude toward the elderly and disabled
- Must be at least 18 years of age
- Must be able to speak, read, write, and understand English
- Must be capable of performing the Essential Job Functions and Physical and Sensory Requirements for All Job Functions as outlined below.
Standard Requirements
- Support the Mission, Values, and Vision of NHS Management, L.L.C. and the facility.
- Knowledgeable of resident rights and ensures an atmosphere, which allows for the privacy, dignity, and well-being of all residents in a safe, secure environment.
- Report to Management immediately all instances of abuse, neglect, misappropriation/handling of resident property, or other concerns such as inadequate staffing, etc.
- Participate in all investigations as required.
- Follows policies for acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.
- Support, cooperate with, and implement specific procedures and programs for:
- Safety, including standard precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.
- Maintain confidentiality of all data, including resident, employee, and operations data and comply with HIPAA Privacy and Security.
- Comply with all Quality Assurance and regulatory requirements.
- Comply with current law and policy to provide a work environment free from harassment, and all illegal and discriminatory behavior.
- Complete required in-service training.
- Support and participate in common teamwork and provide great customer service
- Have a positive frame of mind, friendly communication and genuine care and concern with residents, families, visitors and co-workers.
- Treat residents, families, visitors and co-workers with courtesy and respect, say please, thank you, hello, and goodbye.

Essential Job Functions
- Assist in planning, developing, organizing, implementing, evaluating, supervising and directing the social services programs and activities.
- Participate in an ongoing quality assurance program as directed by the Social Services Director.
- Maintain up-to-date resident right reviews and advance directives with resident and/or responsible party according to Federal and State guidelines.
- Complete necessary forms, reports and progress notes as directed by the Social Services Director.
- Participate in community planning, development and implementation of social care plans, resident assessments and discharge planning to meet the needs of the facility, the residents and their families.
- Assist in coordinating the social services programs with the residents, their families, staff members and community agencies through interviews, consultations and direct involvement in the programs.
- Provide information to residents and their families concerning available support agencies and financial assistance programs to meet the needs of the residents.
- Assist with the admission process such as notifying staff of admission, completing admission documents, facility tours, phone inquiries, etc.
- Maintain a professional, respectful relationship with any community contacts made in the referring process.
