Clinical Screener-Subacute/Skilled Nursing experience

North and Central NJ area

Hybrid position-transition to fully remote

We are looking for an experienced Clinical Screener for our Subacute/Skilled Nursing facilities. In this role you will train in the center, with transition to fully remote status and occasional flexibility and willingness to work onsite. The Clinical Screener will have responsibility to assess and clinically evaluate referrals for clinical appropriateness for admission to the facility. The Clinical Screener will communicate directly with admissions/intake and clinical and administrative leadership at the faciity to support the admission process via the completion of a clinical screen, identification of applicable equipment needs, and reviewing of medications to ensure sound fiscal decision-making around acceptance, enter clinical assessment into UDS IRF platform, while ensuring expedient response to referral sources and optimal care transitions. Manage clinical qualification of referrals received by hospital accounts with the goal of expedited response to referral sources that support conversion to admission.

License/Certification:
•LPN, RN, SW, OTA, PTA, OT, PT

Experience:

• Clinical licensure of the form above
• Subacute/Skilled experience preffered
• Ideal but not required experience within an IRF and working knowledge of capabilities
• Demonstrated technical skill and ability to navigate and communicate within referral platforms, Electronic Medical Records and UDS Platform
